This fictional case study explores “Jeremy’s story” to highlight the transformative power of acceptance and commitment therapy (ACT) in addressing the emotional and behavioural challenges that often accompany physical health conditions. Through an introduction, formulation and then intervention it looks at the different ways an ACT framework can help us support clients.

As a building site manager, Jeremy’s life was deeply impacted by a back injury that forced him to step away from his role temporarily. On returning to work, he found himself in a position he felt ill-suited for, which further compounded his sense of frustration and loss. Struggling to cope, Jeremy turned to avoidance behaviours, withdrew from his loved ones, and relied increasingly on alcohol as a way to manage his emotional pain.
Through ACT, Jeremy learned to accept the reality of his altered physical health and began to nurture healthier coping mechanisms. This shift allowed him to break free from the cycle of avoidance and disconnection, ultimately creating a more compassionate relationship with himself and those around him. Jeremy’s journey demonstrates how ACT can empower individuals to embrace change, align with their values, and rebuild a life rich in connection and purpose, even in the face of significant challenges.
